Individual Checklist
Make sure that you have documentation/records for each of the items below.
- Group certificates, Workers Compensation and pension and unemployment certificates
- All eligible termination payments including Rollover advices.
- Investment or any other Income
- Interest, rent, dividends and franking credits.
- Jury duty fees.
- Details of any sale of assets (shares, real estate, etc.)
- Motor Vehicle Expenses - Car Mileage attributable to work/business, type of vehicle- include engine capacity,log books & receipts where applicable.
- Work related travel costs that have not been reimbursed
- Uniforms or protective clothing
- Work related self education costs
- Other - memberships seminars etc
- Receipts for donations.
- Details of medical expenses if they exceed $1,500 in total: Medical, dental, hospital, optometrist, etc. net of refunds received.
- Details of personal disability insurance and superannuation contributions including details of employer superannuation support.
- Details of any changes in dependants including spouse income
- Details to support any claim for Medicare Levy exemption, i.e. low income earners, health care card holders etc.
- Have you changed your place of residence during the year.
- Are you or your spouse in receipt of any Government Payment e.g. Parenting Allowance (if so enclose certificates)
- Superannuation contributions for a spouse who is not working or has less than $10,800 assessable income.
- Details of Private Health Cover
- Rental property expenses including rates, interest, body corporate fees, insurance, commissions, rental agents summary statements and repairs.
- Superannuation - paid by you personally (non employer contributions)
